Incidence and case fatality of stroke in Korea, 2011-2020

Abstract
OBJECTIVES
Stroke remains the second leading cause of death in Korea. This study was designed to estimate the crude, age-adjusted and age-specific incidence rates, as well as the case fatality rate of stroke, in Korea from 2011 to 2020.
METHODS
We utilized data from the National Health Insurance Services from January 1, 2002 to December 31, 2020, to calculate incidence rates and 30-day and 1-year case fatality rates of stroke. Additionally, we determined sex and age-specific incidence rates and computed age-standardized incidence rates by direct standardization to the 2005 population.
RESULTS
The crude incidence rate of stroke hovered around 200 (per 100,000 person-years) from 2011 to 2015, then surged to 218.4 in 2019, before marginally declining to 208.0 in 2020. Conversely, the age-standardized incidence rate consistently decreased by 25% between 2011 and 2020. When stratified by sex, the crude incidence rate increased between 2011 and 2019 for both sexes, followed by a decrease in 2020. Age-standardized incidence rates displayed a downward trend throughout the study period for both sexes. Across all age groups, the 30-day and 1-year case fatality rates of stroke consistently decreased from 2011 to 2019, only to increase in 2020.
CONCLUSIONS
Despite a decrease in the age-standardized incidence rate, the total number of stroke events in Korea continues to rise due to the rapidly aging population. Moreover, 2020 witnessed a decrease in incidence but an increase in case fatality rates.

Abstract
OBJECTIVES
Cardiovascular diseases are a leading cause of mortality worldwide, and acute myocardial infarction (AMI) is particularly fatal condition. We evaluated the incidence and case fatality rates of AMI in Korea from 2011 to 2020.
METHODS
We utilized data from the National Health Insurance Services to calculate crude, age-standardized, and age-specific incidence rates, along with 30-day and 1-year case fatality rates, of AMI from 2011 to 2020. Age-standardized incidence rates were determined using direct standardization to the 2005 population.
RESULTS
The crude incidence rate of AMI per 100,000 person-years consistently increased from 44.7 in 2011 to 68.3 in 2019, before decreasing slightly to 66.2 in 2020. The age-standardized incidence rate of AMI displayed a 19% rise from 2011 to 2019, followed by a slight decline in 2020. The increasing trend for AMI incidence was more pronounced in males than in females. Both 30-day and 1-year case fatality rates remained stable among younger individuals but showed a decrease among older individuals. There was a minor surge in case fatality in 2020, particularly among recurrent AMI cases.
CONCLUSIONS
Over the past decade, the AMI incidence rate in Korea has consistently increased, with a slight downturn in 2020. The case fatality rate has remained relatively stable except for a minor increase in 2020. This study provides data for continuous surveillance, the implementation of targeted interventions, and the advancement of research aimed at AMI in Korea.

Abstract
Ebola virus disease (hereafter Ebola) has a high fatality rate; currently lacks a treatment or vaccine with proven safety and efficacy, and thus many people fear this infection. As of August 13, 2014, 2,127 patients across four West African countries have been infected with the Ebola virus over the past nine months. Among these patients, approximately 1 in 2 has subsequently died from the disease. In response, the World Health Organization has declared the Ebola outbreak in West Africa to be a Public Health Emergency of International Concern. However, Ebola is only transmitted by patients who already present symptoms of the disease, and infection only occurs upon direct contact with the blood or body fluids of an Ebola patient. Consequently, transmission of the outbreak can be contained through careful monitoring for fever among persons who have visited, or come into contact with persons from, the site of the outbreak. Thus, patients suspected of presenting symptoms characteristic of Ebola should be quarantined. To date, South Korea is not equipped with the special containment clinical units and biosafety level 4 facilities required to contain the outbreak of a fatal virus disease, such as Ebola. Therefore, it is necessary for South Korea to make strategies to the outbreak by using present facilities as quickly as possible. It is also imperative that the government establish suitable communication with its citizens to prevent the spread of uninformed fear and anxiety regarding the Ebola outbreak.

에볼라 바이러스병은 치명률이 높고 효과와 안전성을 모두 검증받은 치료제나 백신이 없어 많은 사람들이 두려워하고 있는 질환이다. 이번에 서아프리카에서 9개월째 지속된 유행으로 8월 13일까지 4개국에서 2127명의 환자가 발생하였고 1145명이 사망하여 2명중 1명꼴인 높은 치명률을 보이고 있다. 이에 WHO에서는 국제보건긴급상황을 선포하였다. 하지만 에볼라 바이러스는 증상이 나타난 이후에 전염되며 환자의 혈액이나 체액에 접촉한 경우에만 감염이 된다. 따라서 유행지역을 방문하였거나 접촉한 사람을 대상으로 발열감시와 함께 환자 격리를 철저히 시행한다면 유행 전파를 막을 수 있다. 한국은 아직 에볼라 바이러스와 같은 질환에 적합한 격리병상이나 실험실 안전 4등급 시설이 없다. 이에 대한 대비를 빠른 시간에 마련하여 대응하는 것이 필요하며, 국민과의 소통을 통해서 막연한 불안과 두려움을 예방하는 것이 무엇보다 중요하다.

Survival Rate and Prognostic Factors of Cancer Patients Diagnosed in a University Hospital.

Abstract
PURPOSE
This study was conducted to investigate case-fatality rates and survival rates, prognostic factors of prevalent five cancers(stomach, lung, liver, cervix, colon) in a university hospital located in Taejon City.
METHODS
2,158 cancer patients who have admitted the hospital from Jan. 1, 1991 to Dec. 31, 1998 were analysed. The higher ratio of outbreak and growing were selected among the investigated cancer data for over 100 of subjects in Korean cancer patient was examined into two method. The one is medical record and the other is affirm a government office for existence or not.
RESULTS
The lung cancer was discovered for the highest fatality rate. The crude 5-year survival rate of all cancer patients was 43.9% and that in male was higher than that in female. The 5-year survival rate of stomach cancer patients were 49.7%, that treated with combined(operation and chemotherapy) were 66.2%, that with metastasis to lymph node were 48.1%, and distant metastasis were 31.9%. Lung cancer patients were 25.9%, that treated with operation were 42.7%, and that with metastasis to lymph node were 29.3%. Hepatoma patients were 25.5%, that treated with operation were 37.8%. Uterine cervix cancer patients were 74.8%, that treated with operation were 95.0%, that with metastasis to lymph node were 83.3%, and distant metastasis 74.8%. Colon cancer patients were 41.8%, that treated with operation were 50.2%, that with metastasis to lymph node were 33.4%. Prognostic factors affecting survival rate among stomach cancer patients were age, operation, operation with cancer chemotherpy, and metastais to lymph node and distant matastasis. Prognostic factors of uterine cervix cancer was age, and that of colon cancer were operation with radiotherapy, metatasis to lymph node and distant metastasis.
CONCLUSION
case-fatality rate in male were higher than that in female, and increased with age. The cancer survival rate of female is high, the high in over 40 years group, and operation is exposed in the highest survival rate, also significant difference in metastasis level.
